COST NOTES

Ask for
the whole bill.

Put the medicine, the care fee, and the costs outside a plan on the same sheet. Then ask which parts change after the opening offer.

A STARTING POINT, WITH TERMS

What can the medicine cost?

There is no useful single price for “peptide therapy.” Ask for a quote for the exact product and the care around it.

Self-pay means you pay for the medicine without using insurance.

For semaglutide (Ozempic, Wegovy) and tirzepatide (Zepbound), many self-pay offers span $149–$499 per monthly supply, as of September 26, 2026.

For certain prescribed strengths of tirzepatide (Zepbound) in small bottles called vials or the injection device called KwikPen, the maker’s Self-Pay Journey Program offers a discount, as checked September 26, 2026.

The discount applies to the first purchase at that strength or a refill purchase within 45 days of the last delivery, as checked September 26, 2026.

Missing the deadline can raise the medicine price to $699, depending on your prescribed strength, as checked September 26, 2026.

Look for “Self-Pay Journey Program” on your quote. Ask whether your prescribed strength gets that discount, what date your refill is due, and what you will pay if you miss it.

The range includes starting offers and varies by medicine, strength, and refill terms.

These are medicine prices; a care membership and tests may add to the bill.

As checked September 26, 2026, a Zepbound month means 28 days of medicine.

For semaglutide (Wegovy), a month means a box of four injection pens or a bottle of 30 tablets, as checked September 26, 2026.

For semaglutide (Ozempic) shots, the maker calls a box with one injection pen a month’s supply, as checked September 26, 2026.

Ask for the number of pens, bottles, or tablets, how many days the medicine will last, and how often you will be charged.

Basis: the makers’ published self-pay terms.

Many savings-card and cash-offer terms expire on December 31, 2026, as checked September 26, 2026.

Get the offer’s end date in the written quote.

Will insurance pay?

Start with the exact drug and the reason it is prescribed. Ask your plan about its list of covered medicines and any approval needed before it pays.

Medicare drug plans can cover some of these medicines for another health problem, such as type 2 diabetes, as of September 26, 2026.

Another possible use is lowering heart-attack and stroke risk in people with heart disease and excess weight, as of September 26, 2026.

That still depends on the plan’s covered-drug list and any prior approval it requires.

Prior authorization is the insurer’s step for agreeing to pay in advance.

YOUR COST QUESTION SHEET

What is still missing from the quote?

Choose what the offer tells you. Use the questions that follow when talking to the service or your insurer.

What is clearly written down?

Price is one part of the decision.

Keep the written quote with the follow-up plan and the names of both the clinician and the pharmacy. A missing answer stays a missing answer, however appealing the price looks.
